July 17, 1816. List of 48 items, each with price, totalling over 32 dollars. For Dr. Ezra Gustin bought of Thomas Prather (?) Some of the medicines & medical supplies listed incl. sulphur ether, syringes, vials, lancets, borax soda, much else. On reverse after total is a MS paragraph relating to delivery of a full supply of an item in a month.
