London: Edward Arnold and New York Samuel Buckley & Co., 1903. Number 114 of 150 copies, all printed on vellum. Printed at The Essex House Press, Campden, Gloucestershire, with a frontispiece by W. Strang. Small Octavo, original vellum with gilt lettered spine, and with Arts and Crafts rose in blind with "Soul is Form" in blind. Near fine with light foxing on rear cover; ex-libris bookplate of prior owner on front pastedown.
