Page 5 is the official decree from the Presidium of the Supreme Soviet of the USSR revoking the citizenships of both Josyp Mikhaylovich and Elena Timofeyivna Terelya followed by, on page 6, the decree from Alexis, Volodar and King of Ukraine-Ruthenia, admitting Josyp to the Military and Hospitaller Order of Saint Lazarus of Jerusalem with the rank of Knight Grand Cross of Justice. One page preface by Roman Danylak, the titular bishop of Nyssa, dated 2001 and then followed by a 6 page introduction by Roman Chrucky. The book appears to have never been used. .
