The Artisans Order of Mutual Protection, established in 1873, is the second oldest fraternal insurance organization in the United States. In 1937, as described here, membership was available to white men between 18 and 50 years, "physically sound, at least five feet in height, 100 pounds in weight, and who believes in the existence of a Supreme Being." This form was intended for membership in the Enterprise Assembly and gives the Philadelphia addresses of the recorder, Geo. F. Johnson, and medical examiners, Elmer F. Keiser, Geo. F. Enoch, and Wallace Martin. Includes a list of occupational and other hazards not admitted and those admitted at higher rates. A wide variety of occupations were not accepted from aeronauts to racers of cars, bicycles, and horses to both boxers and wrestlers. Hatters and officers of the law were admitted at highter rates.
